JOB SUMMARY The Community Health Worker (CHW) serves as a patient-facing member of the Population Health and Value-Based Care team and functions as a bridge between patients, healthcare providers, care management staff, and community resources. The CHW works collaboratively with nurses, social workers, physicians, care managers, behavioral health professionals, dietitians, diabetes educators, and other members of the interdisciplinary team.
