Maintains accurate program and patient documentation, including medical record, develops andimplements a plan of treatment, in conjunction with members of multi-disciplinary team, conducts groups, Communicates with referral sources and other appropriate parties regarding patients' progress, Coordinates patients' discharge planning and implementation. Operating acute care hospitals, behavioral health facilities, outpatient facilities and ambulatory care access points, an insurance offering, a physician network and various related services located all over the U.S. states, Washington, D.C., Puerto Rico and the United Kingdom.
