CHASI provides outreach, education, prevention, and direct support services for populations most affected by health disparities - people with low or no income, low-income people with chronic illnesses, people with criminal justice involvement, people who use drugs, domestic violence survivors, people of color, and the LGBTQ community. Maintain ongoing face-to-face, telephone, and other collateral contacts to support, encourage, and promote the client's commencement and ongoing compliance with the requirements of treatment and/or services outlined by CHASI and the funder, as well as to assist with mitigating barriers to engagement in services, wellness, and recovery, as well as employability by working closely with the treatment team.
