Actively participates in the coordination of care services, evaluating the ongoing needs of residents (attends resident care plan meetings, rehabilitation and resident/family meetings, departmental staff meetings, annual policy review meetings, monthly quality assurance meetings, etc.). Serving families throughout San Joaquin County, we provide short-term rehabilitation and long-term care focused on faster recovery, stronger outcomes, and a truly supportive environment.
