p>Posting dates for internal transfers: April 17, 2026, through April 19, 2026 BASIC FUNCTION: Under the direct supervision of the Program manager and Clinical Coordinator of Wound Care Department, provides patient with care that is continuously evaluated, planned for, and revised as needed; patients status is documented within the reassessments; patient care needs; interventions to meet the patients needs; nursing care provided; the patients response to; and the outcome of care provided; organizes and prioritizes work to provide health information, discharge planning, and patient care in a safe environment to maximize health in the continuum of care and protect the patient; assumes responsibility for and assigned group of patients and be accountable for an assessment of the patients basic human needs, enabling team of providers to make appropriate decisions to develop, implement and evaluate a plan for care, to include biophysical needs, bio-psychological needs, environmental/safety needs, self-care needs, educational needs, discharge planning needs; professional and competent as a care provider, a leader of personnel, and a collaborator with other professionals; demonstrates awareness of his/her unique position in the Department and responsibilities as a professional nurse. Must have current knowledge/training in wound care possess or willing to become certified in wound care.
