Specific duties of the role include, but are not limited to:- conduct health assessments to identify patient needs- develop personalized care plans- regularly evaluate patient conditions and responses to treatment- adjust treatment plans as necessary- perform minor procedures and wound care as required (and as settings permit)- educate patients and families on health management, disease prevention, etc.- provide guidance on lifestyle modifications and self-care practices- work with physicians, nurses, and other professionals to coordinate careThe practice uses a mobile-based EHR with AI functionality to reduce administrative time.
