This role is ideal for an experienced PMHNP who enjoys working in a collaborative outpatient environment alongside therapists and who values thoughtful, client-centered psychiatric care. Founded in 2023, Tides Mental Health is a growing private practice dedicated to providing exceptional mental health care to individuals, couples and families in our community.
p>Provides screening, preventive primary care and medical care services to members - primarily in non-clinical settings where members feel most comfortable, including in-home, community and nursing facilities and "pop up" clinics.
In collaboration with Upperline's local Physician team, APPs develop complex plans of care in accordance with the patients health status and overall goals and values; provide medical diagnostics and treatments in the clinic, in the patients home, or via telehealth, consistent with the APP's scope of practice and the patients medical condition; educate patients and families on medication usage, diabetes technology (as applicable), lifestyle modification, diet & nutrition, and crisis anticipation & prevention. Upperline Health providers coordinate patients care among a team of specialists, Physicians, Advanced Practice Providers, Care Navigators, Pharmacists, Dieticians, Podiatrists, Endocrinologists and Social Workers for integrated treatment that addresses patients immediate and long-term health needs.

The Nurse Practitioners will partner with the Medical Directors, Chief Medical Officer, and the medical team to assure the highest quality of patient care, including comprehensive evaluations, patient consultations, and ensuring compliance with medical and clinical policies and procedures.
