This role is ideal for an experienced PMHNP who enjoys working in a collaborative outpatient environment alongside therapists and who values thoughtful, client-centered psychiatric care. In collaboration with Upperline's local Physician team, APPs develop complex plans of care in accordance with the patients health status and overall goals and values; provide medical diagnostics and treatments in the clinic, in the patients home, or via telehealth, consistent with the APP's scope of practice and the patients medical condition; educate patients and families on medication usage, diabetes technology (as applicable), lifestyle modification, diet & nutrition, and crisis anticipation & prevention.
