conduct initial and periodic nutrition assessments in the LIFE Center, participant homes, hospitals, and skilled nursing facilities; evaluate intake, medical conditions, medications, weight patterns, and cultural dietary preferences; monitor high‑risk concerns such as malnutrition, weight loss, dysphagia, and wound healing needs; provide nutrition counseling to participants, caregivers, and staff on special diets, healthy eating, and meal preparation; and. In this role, you'll manage the nutritional needs of older adults with complex medical and functional challenges, supporting participants across multiple settings—including the LIFE Center, skilled nursing facilities, hospitals, and participant homes—to improve health outcomes, maintain functional ability, and enhance quality of life.
