conduct initial and periodic nutrition assessments in the LIFE Center, participant homes, hospitals, and skilled nursing facilities; evaluate intake, medical conditions, medications, weight patterns, and cultural dietary preferences; monitor high‑risk concerns such as malnutrition, weight loss, dysphagia, and wound healing needs; provide nutrition counseling to participants, caregivers, and staff on special diets, healthy eating, and meal preparation; and. In this role, you"ll manage the nutritional needs of older adults with complex medical and functional challenges, supporting participants across multiple settings-including the LIFE Center, skilled nursing facilities, hospitals, and participant homes-to improve health outcomes, maintain functional ability, and enhance quality of life.
