Act as a Liaison between area schools, school-based health clinics, integrated behavioral health, social health, and local pediatric providers, establishing regular connections with designated school staff; facilitate involvement on campus and improve outreach to local student populations. Evaluate the patient/client’s cognitive, communicative, and swallowing status and develop an individualized plan of care with measurable treatment goals and intervention methods appropriate to the patient's age, condition, diagnosis, response to treatment, and learning needs.
