Partner with the Medical Director to develop and implement clinical protocols, collaborate with hospital systems, payers, and community organizations, and attend leadership meetings. Lead an interdisciplinary care team of physicians, nurses, behavioral health specialists, medical assistants, care managers, and community health workers.
