We’re a mission-centered outpatient clinic in the NYC metro area built to deliver whole-person primary care for patients who too often fall through the cracks, people facing housing insecurity, complex medical needs, and barriers to consistent access. Our model is intentionally integrated and team-based: primary care is supported by behavioral health collaboration, care management, and strong operational partners, so clinicians can practice thoughtful medicine with real support, not heroics.
