This role is accountable for the daily operations, quality outcomes, staff performance, and regulatory compliance of services supporting patients transitioning across the care continuum, including ED-to-Home- accelerated outpatient management, chronic condition follow-up, and Intense Care Coordination programs. The Nurse Manager ensures safe, effective, and patient-centered care delivery; fosters interdisciplinary collaboration; ensures timely follow-up and care coordination; and advances Mercy’s mission through high-quality virtual care management and population health strategies.
