As a member of our interdisciplinary team, you will be responsible for performing psychosocial assessments, providing supportive counseling, and care coordination with respect to discharge planning and service coordination for adult, geriatric, pediatric, and med/surg populations. Experience with protective services, child welfare, long-term care, crisis intervention, chronic illness, hospice care, substance use disorders, and psychiatric illnesses is a plus.
