In this role, you will be a key member of our multidisciplinary team, delivering comprehensive medication management and contributing your expertise to Saint Luke's Population Health initiatives.
Key Responsibilities: Medication Management & Dispensing: Prepare and dispense medications accurately, interpret medication orders for appropriateness, and maintain secure medication distribution systems in accordance with internal policies and regulatory bodies. Conduct and document pharmacokinetic/pharmacodynamic consultations, provide comprehensive medication education to patients and caregivers, and ensure cost-effective care delivery.
Should be involved in special education programs for patients as to the types of drugs used, why and how they are used and the importance of following directions and the importance of home lab tests, e.g., urine, etc. Should develop a system for review of medications used to inform the patient of potential side effects, proper methods of taking medications and possible undesirable, but normal or expected reactions.
