This position reports to the Buffalo, MN office and provides services in Elk River, Otsego, Rogers, Maple Grove, Big Lake, Becker, Zimmerman, Princeton, Monticello, St. Michael, Albertville and surrounding cities. Performs initial and periodic assessments of clients to determine care needs, implements interventions, and evaluates the effectiveness of care to achieve positive outcomes.
