Position Location Closes Part Time Care Coordinator, Center for Counseling & Student Development Newark, DE Nov 20 2026 Under the direction of the Assistant Director for Care Coordination, the Care Coordinator provides care navigation services to students needing resources outside of CCSD for routine, urgent, and emergent concerns. Primary Responsibilities: Establishes relationships with on-campus and community partners to gain awareness of the intake, discharge process for hospitals and intensive outpatient programs (IOPs), and knowledge of services provided by various on-campus partners including Student Health Services, Student Wellness and Health Promotion, and other offices when appropriate.
