Part Time 1pm-9:30pm with every other weekend.The Patient Access Specialist reflects the mission, vision, and values of NMHC, adheres to the organizations Code of Ethics and Corporate Compliance Program, and complies with all relevant policies, procedures, guidelines and all other regulatory and accreditation standards.Responsibilities:<ul><li>Consistently practices Patients First philosophy and adheres to high standards of customer service. This includes setting an example to peers, coworkers, etc. by fostering a team atmosphere.</li><li>Responds to questions and concerns.</li><li>Forwards, directs, and notifies Team Lead or Operations Coordinator of extraordinary issues as necessary.</li><li>Maintains patient confidentiality per HIPAA regulations.</li><li>Provides exceptional customer service to patients which establish a positive first impression of Northwestern Medicine.</li><li>Exceeds all consumer requests and alerts management of issues or concerns that require escalation.</li><li>Correctly identifies and collects patient benefit levels through the use of online tools (NDAS, ASF, etc.) or over the phone as necessary.</li><li>Completes accurate handoff instructions and notes to scheduling staff, by noting appropriately in Epic.</li><li>Demonstrates ability to use all computer applications efficiently and to the capacity needed in this position.</li><li>Runs real time eligibility (RTE) on all patients to verify insurance and follows out of network policies as applicable.</li><li>Sends quality Epic Messages/Telephone encounters that are descriptive and grammatically correct.</li></ul>Efficiency, Process  How you make a difference:
The Licensed Practical Nurse (LPN) plays a vital role in patient care by contributing to the nursing process of assessment, planning, implementation, and evaluation. Working under the supervision of RN(s), the LPN provides direction and guidance in patient education and activities that align with their education and demonstrated competencies. 

 Key Responsibilities:
<ul><li>Under RN supervision, assesses patients' physical, psychological, and social dimensions in both the Health Care Unit and housing units, contributing to individualized treatment planning and consulting with RNs and staff as needed.</li><li>Implements treatment programs directed by Health Care Practitioners, administering medications, including IV therapy (if certified), and performing diagnostic tests according to established protocols and procedures.</li><li>Collaborates in controlled substance counts at the beginning of each shift, adhering to safety techniques, procedures, and standard precautions while responding to codes or health emergencies within standard guidelines.</li><li>Assists in medical or minor surgical procedures, implements nutrition and therapeutic diet plans, and documents nursing encounters using SOAP charting, ensuring effective communication with nursing staff, physicians, and other relevant personnel.</li><li>Attends mandatory staff meetings and training sessions, demonstrating a commitment to continuous professional development and adherence to policies and procedures in providing high-quality patient care.</li></ul>

 Additional Details:
We are looking for a part time LPN to join our clinical team at McHenry County Jail. This position is 16 hours a week on the 2p-10:30p shift and every other weekend. 

 Qualifications & Requirements:
Education<ul><li>Graduate from an accredited school of nursing.</li></ul>Experience<ul><li>Prefer a minimum of one (1) year clinic experience</li></ul>Licenses/Certifications<ul><li>Have and maintain current licensure as a Licensed Practical/Licensed Vocational Nurse within the state of employment.</li><li>Must be able to obtain and maintain CPR certification.</li></ul>
