Reviews, maintains, and screens patient medication orders and profiles for allergies, drug-drug interactions, drug-food interactions, contraindications, potential adverse drug reactions, therapeutic duplication, and out of range doses. Communicates with physicians, nurses, and other healthcare providers and patients to intervene on orders that are unclear, incomplete, inappropriate, or could cause drug misadventures and recommends alternative therapy.
