Training and experience,and/ordemonstrated competency in casework principles,includingdiagnosis,assessment, crisis intervention, treatment and transition planning; techniques in individual, family andgroup therapy; awarenessof community resources, public assistanceand entitlementprograms,ensuring transition to appropriatelevels of care; and issues relating to age and stage ofdevelopment, special needsand cultural patternsof the patientpopulationsbeing served. Responsibilities: (30%)* Provide social work interventionas it relates to hospitalizationincluding crisis management, health care decision making, illnessadjustment,ethical/legal concerns, dischargeplanning,and transitional care needs, child or elderabuse, domestic violence, competency, financial problems, compliances issues, substance abuse, mental illnessaswell as otherpsychosocialbarriers to maximizing health status.
