This position focuses on facilitating groups, treatment planning, discharge planning, care coordination, care conference meetings, communication with caretakers, community providers, and others involved in the care of our patients. Assess, plan, implement, and evaluate the psychosocial services for patients and share social work expertise with members of the treatment team within the average length of stay of 30-90 days.
