Through Casa’s ITS Model (individualized and integrated care model), the Nurse Manager will work collaboratively with our Medical Director, NP Medical Director Designee (FNP), and our PMHNP, to lead our nursing team, provide the highest quality patient care, implement doctor’s orders, monitor specified medication/refills, monitor and record vital signs as needed, and maintain thorough and timely medical/nursing records. Casa Esperanza’s Conexiones program is a 32-bed facility that provides Individualized Treatment Services Program (ITS) which delivers integrated acute treatment services and withdrawal management and clinical stabilization services (CSS) to adults struggling with co-occurring substance use and mental health treatment needs.
