The Clinical Pharmacist is responsible for the provision of clinical pharmacy services including: anticoagulation, pharmacokinetics, antibiotic stewardship program, drug dosing consultation, nutritional support monitoring, drug information, formulary reviews, in-services on pharmacology/disease state pharmacotherapy, pharmacokinetics to medical, nursing, and pharmacy staff. This includes the selection of medications to disease state, monitoring of patient’s therapeutic outcomes, follow-up on medication errors and evaluation of adverse drug reactions.
