ul>Develop and maintain an ambulatory care practice that includes, but is not limited to, the following collaborative activities with the patient care team:
Conduct patient assessments and comprehensive medication reviews to identify, resolve, and prevent drug therapy problems;
Formulate medication treatment plans;
Adjust medication doses based on pharmacokinetic and pharmacodynamic principles;
Initiate or modify drug therapy using established therapeutic plans or protocols;
Discontinue medications that are no longer required;
Administer select medications and vaccinations as permitted by state law;
Identify, prevent, and document adverse drug events and medication errors;
Participate in cardiopulmonary resuscitation efforts, as appropriate;
Document the care provided and communicate essential information to other care providers;
Coordinate and integrate medication management into the patient''s overall health care plan;
Provide other patient care services as permitted by the West Virginia Pharmacy Practice Act.
